Chronic Knee Pain is common in old age and the most common cause is OsteoArthritis.
In osteoarthritis, the joint cartilage and the surrounding structures of the knee joint become rough and weak. The space between the thigh bone (femur) and the leg bone (tibia) becomes less. That’s why the bones start rubbing together when the patient walks and hence the pain occurs.
An X Ray would confirm the presence of osteoarthritis and also tell the degree of the damage.
If the arthritis is mild, then doing special exercises of the knee will increase the strength of the surrounding muscles. Once the muscles are strong, they take most of the weight and the pressure on the joint decreases. Reducing body weight will also help if patient is overweight.
Targeted anti-inflammatory injections use a special medicine that stays in the joint for 2-3 months and give good relief. The patient should exercise regularly in this time so that the muscles strengthen and the pain does not reappear.
Platelet Rich Plasma is the injection of platelets separated from the patient’s own blood. The blood is withdrawn from any arm vein, put in a special machine, and the platelets are separated. Once these platelets are injected in the joint, they start the normal natural healing process. This injection has no side effects and gives a very long term relief.
Genicular nerves are small nerves that take the pain sensation from the knee joint and take it to the brain. They are small nerves and do not take part in giving power to the leg muscles. A thin needle is inserted and put very close to the nerve. A very small current is passed which is felt like a tingling to the patient. This deactivates the nerve and gives relief for 1-2 years. Side effects can be only a very mild sense of numbness in a small area around the knee.
Visco-supplementation is the injection of a lubricating material in the knee joint. It stays in the joint for few months and prevents the bones from rubbing against each other.
